- に基づいた — modifies a following noun directly — 実話に基づいた映画 "a movie based on a true story"; に基づいて cannot sit in that slot. The dictionary-form に基づく reads more generic (事実に基づく報道 "reporting grounded in facts").

Both require the noun to be a fixed reference the action stays faithful to, but に沿って's reference is a COURSE — a plan, guideline, or literal path the action tracks moment by moment without straying (計画に沿って進める) — while に基づいて's reference is a standard or piece of evidence a single judgment or product is built to be consistent with (証拠に基づいて判断する); に沿って reads as ongoing tracking along a line, に基づいて as one-time grounding in a source.

Both name a source that grounds what follows, but 基づいて demands the result stay FAITHFUL and consistent with the source — a ruling grounded in evidence, a film sticking close to real events (実話に基づく映画) — while 基に treats the source as raw material free to be reshaped (経験を基に小説を書く, changing names and events); 基に's product need not match its source the way 基づいて's must.
